Mediastinal gray zone lymphoma.

چکیده
Introduction The current classification of lymphoid neoplasms is based on the integrated utilization of morphological, immunohistochemical, genetic and clinical criteria to define disease entities, which putatively have their origin in a “normal counterpart” in the immune system. The application of modern immunophenotypic and molecular genetic techniques in routine diagnosis has made it possible for most lymphomas to be successfully diagnosed and classified as one of the currently recognized distinct disease entities. Nevertheless, there are cases that do not follow this principle and show overlapping or borderline morphological, biological and clinical features between various types of lymphomas. The updated 2008 World Health Organization (WHO) classification of tumors of hematopoietic and lymphoid tissues recognized this problem and introduced two new provisional categories of B-cell lymphoma, unclassifiable, one with features intermediate between diffuse large B-cell lymphoma (DLBCL) and Burkitt’s lymphoma and the second, which is the topic of this review, with features intermediate between DLBCL and classical Hodgkin’s lymphoma (CHL). This second provisional category recognizes cases that usually occur in the mediastinum and have features of both nodular sclerosis CHL (NSCHL) and primary mediastinal large B-cell lymphoma (PMBL). In this review PMBL will be briefly considered in order to provide the framework for understanding the concept of mediastinal gray zone lymphoma.

Clinical, pathological and genetic features of primary mediastinal large B-cell lymphomas and mediastinal gray zone lymphomas in children.
UNLABELLED Background Primary mediastinal large B-cell lymphoma is a rare lymphoma accounting for no more than 3% of all B-cell lymphomas in children and adolescents. However, patients in this young age group with this lymphoma have the shortest event-free survival of patients with any B-cell lymphoma under current standard chemotherapy protocols. Lymphomas with features intermediate between pr...

Primary mediastinal B-cell lymphoma (PMBL) is a subtype of diffuse large B-cell lymphoma (DLBCL) that is putatively derived from a thymic B cell. Accounting for up to 10% of cases of DLBCL, this subtype predominantly affects women in the third and fourth decades of life. Its clinical and molecular characteristics are distinct from other subtypes of DLBCL and, in fact, closely resemble those of ...

A 50-year-old male presented with cough and breathlessness. A positron emissoin tomography scan revealed FDG (Fluorodeoxyglucose) avid mediastinal mass. Tru-cut biopsy showed fibrotic stromal tissue with cellular infiltrate consisting of abnormal lymphoid cells and few large cells with smudged nucleus. Immunohistochemistry revealed diffuse positivity with CD20, focal positivity for CD30 and rar...

Mediastinal gray zone lymphoma, B-cell lymphomas with intermediate features between classical Hodgkin lymphoma and primary mediastinal B-cell lymphoma, have not been well described in the literature. We report the clinical characteristics and outcomes of a large retrospective series of 99 cases centrally reviewed by a panel of hematopathologists, with a consensus established for the diagnosis. ...

Mediastinal B-cell lymphomas present in the mediastinum and are most frequent in young patients. Nodular sclerosis Hodgkin lymphoma (NSHL) and primary mediastinal B-cell lymphoma (PMBL) are the common types, whereas mediastinal gray-zone lymphoma (MGZL) is extremely rare and has pathological features intermediate between NSHL and PMBL. The indeterminate pathobiology of MGZL has led to uncertain...
